The case manger functions as the coordinator of the treatment team including but not limited to the patient, family, physician, clinical staff, outside services, payer, and post hospital services. The Case Manager is responsible for the day-to-day operations of the coordination of patient care, including high quality, cost effective delivery of patient care for his/her assigned case load. Under the direction of the Case Management Lead and/or DCM, the Case Manager participates in the quality improvement process for the department in accordance with the overall hospital’s quality improvement plan.

Daily functions include leading the coordination of the treatment team consisting of the patient and family, physician(s) and clinical staff during the delivery of care, and provides appropriate case follow-up to facilitate optimal patient outcomes, management of LOS, appropriate utilization of services and care, and medical necessity compliance. As directed by the DCM and/or Case Management Team Lead, the Case Manager participates in patient specific meetings and hospital established operational and quality meetings.

Minimum Work Experience

  • One (1) year recent relevant experience required.
  • Acute care setting preferred.

Required Licenses/Certifications

  • Current clinical licensure strongly preferred

Required Skills/Knowledge/Ability

  • Demonstrates an understanding of treatment costs and financial support as they relate to quality and efficiency.
  • Utilizes the appropriate leadership skills in delegating, organizing, and educating coworkers and staff.
  • Knowledge of community resourced to meet post-discharge clinical and social needs.
  • Knowledgeable about accreditation standards to ensure adherence to all standard set forth by the state and accrediting agencies of TJC and CMS.
  • Ability to demonstrate clinical proficiency utilizing policies, procedures, and guidelines in caring for patients.
  • Proficient use of appropriate forms in documenting care and medical necessity.
  • Demonstrated organizational skills.
  • Ability to read, write and comprehend English at a level necessary to follow written and oral instructions and safety precautions
