Mountain Valley Home for Health a division of Mountain Valley Regional Rehabilitation Hospital, is currently looking for an Home Health Services Director - FT, to join our Home Health Team. The successful candidate will possess a high degree of internal motivation, the ability to work with minimal supervision and be a team player who wants to excel at assisting patients return to functionality as they transition back to their community. The ideal candidate will have experience in home health.

Mountain Valley Regional Rehabilitation Hospital is a 44-bed, Rehabilitation Hospital. Our facility has been uniquely designed solely for the provision of specialized rehabilitative care and fosters a comfortable and responsive environment for our patients. Our hospital provides rehabilitative services for patients with functional deficits as a result of debilitating illnesses, or injuries. Through our inpatient rehabilitation programs, we help patients recover quickly and return home with a renewed sense of self-sufficiency.

Position Overview:

Under the direction of the Home Health Administrator, supervises home health services to patients in their place of residence in accordance with attending physician orders and plans of care and strives to provide the highest quality of care. The Director of Clinical Services is available at all times during operating hours (or appoints a similarly qualified alternate) and participates in all activities relevant to professional services furnished, including the development of qualifications and assignment of personnel.

Job responsibilities include but are not limited to:

  • Participates in developing standards which ensure safe and therapeutically effective service to patients and families. Has joint responsibility with the Administrator for seeing that standards are met.
  • Is responsible for recruiting, hiring, evaluating and terminating service personnel.
  • Accepts responsibility for regular attendance and punctuality fulfills job requirements without regard to time involved.
  • Oversees Agency's ongoing performance improvement (PI) program.
  • Ensures compliance with federal, state, accrediting bodies, local and Agency policies in all patient care aspects of the Agency.
  • Works under the direction of the Home Health Administrator.

Required Skills:

  • Three to five years of experience in health care/home care,
  • Two years experience in a supervisory position, preferred.
  • Knowledge and ability to apply community health principles and
  • Ability to supervise, guide and develop skills and performance of service
  • Ability to exercise independent judgment.
  • Ability to enlist cooperation of many people in furthering a
  • Ability to deal effectively with high levels of stress.

Required Licenses/Certifications:

  • Licensed as a Registered Nurse (Bachelor degree or higher preferred) with ability to apply and maintain RN license in state of Arizona.
  • Current CPR certification.  ACLS certification preferred.
  • Fingerprint certification through state of Arizona
