Job Description

Under the direction of the RN, the Licensed Practical/Vocational Nurse provides professional nursing care for the comfort and well-being of patients through the use of the nursing process and supports American Nursing Association (ANA) standards of nursing care.   Under the direction of a Registered Nurse, the Licensed Practical/Vocational Nurse is responsible for collecting data and patient information, caring out the established plan of care designed to prevent complications of physical disability, restore optimal functioning and help the patient adapt to an altered life style.  Under the supervision of a Registered Nurse, the Licensed Practical/Vocational Nurse is responsible for providing patient care through support personnel.  The LPN/LVN provides care to adolescents through geriatrics.  The goal is met by cooperation and communication with inter- and intra-departmental staff, patients, and families.

Essential Functions:

  • Practice/Summary of findings— According to established standards of care, utilizes a systematic approach to provide quality patient care by collecting relevant data and information.
  • Plan—Assists in developing an individualized plan of care for each patient.

  • Implementation—Implements the identified plan of care

  • Evaluation—Contributes to the evaluation of progress toward attainment of outcomes

  • Documentation—Provides documentation that is accurate, appropriate, and complete.                

  • Delegation and Coordination—Delegates and coordinates patient care to facilitate optimum patient outcomes.

  • Process Improvement—Helps identify problems and potential risks and contributes to corrective action plans.

  • Age Specific Care—Exercises appropriate application of age specific standards.
