Northern Utah Rehabilitation Hospital is a 40-bed, Rehabilitation Hospital.  Our facility, designed solely for the provision of specialized rehabilitative care, is the only freestanding rehabilitation hospital in Northern Utah. The facility’s unique design will foster a comfortable and responsive environment for our patients.

Our hospital provides rehabilitative services for patients with functional deficits as a result of debilitating illnesses, or injuries. Through our inpatient rehabilitation programs, we help patients recover quickly and return home with a renewed sense of self-sufficiency.

Position Overview:

Under the direction of the Director of Case Management, the Case Manager participates, as appropriate, in patient rounds, medical staff seminars, nursing staff conferences, and in conferences with individual physicians, nurses, and other personnel concerned with the care of a patient and the patients family.

Job responsibilities include but are not limited to:

  • Establishes and maintains pro-active relationships with internal interdisciplinary team members, local payers, physicians, community service organizations and healthcare facilities.
  • Leads the Case Conference and ensures team participation. Gathers appropriate information for timely communication with payor case managers.
  • Documents communication and coordination activities in the health record, in keeping with Case Management Guidelines and Standards of Care, allowing team members to follow through with established plans in their absence.
  • Provides timely and accurate communication to the billing department regarding payer agreements. Provides feedback to the clinical teams regarding payer restrictions. Advocates for patient and family with payers and clinical team.
  • Coaches team toward successful completion of patient care plan. Identifies barriers to care plan completion and facilitates problem-solving with involved parties to modify plan for successful resolution.
  • Monitors physician orders for threshold items ordered during inpatient stay. Reports findings and recommendations to the attending provider or Medical Director. Follows up to determine resource availability, reliability and validity of treatment and makes recommendations regarding modification per physician orders.
  • Provides timely team feedback regarding outcomes and discharge follow-up.
  • Utilizes appropriate community resources to assist in completing a successful discharge.
  • Performs utilization review functions with an interdisciplinary team. Communicates collaboratively with the Director of Nursing Operations to resolve issues and develop systems to facilitate the overall functions of the Nursing unit. Informs the DNO of current status of patients.

Required Skills:

  • Three (3) years case management experience preferred.
  • Must be a licensed RN or Social Worker in the State of Utah.
  • Rehabilitation Hospital experience preferred
